Onboarding: collect sizes once, properly

The cheapest moment to get equipment right is before the first order. Structured size collection at onboarding — flight suit, clothing, gloves, footwear where relevant — plus delivery details means a new crew member's first kit arrives complete and fitting, instead of trickling in over the first month.

Issue: record what actually left the shelf

An order record is not an issue record.
